The Care Hub Foundation
Through this work, we have established the core components of a Care Hub—shared systems that support evidence-based prevention programs across Hawaiʻi’s health centers. By strengthening program delivery, data, compliance, reimbursement, training, and partnerships, we are laying the groundwork for more coordinated, sustainable approaches to care.
